Acknowledgement of Receipt *

I hereby acknowledge that I have received, read, and understand Vistancia Pool Care Terms and Conditions setting forth the basis for weekly swimming pool service and/or additional maintenance. I have had all of my questions answered about the Terms and Conditions Agreement. If I choose to have services cancelled, an email or letter will be required for termination of service at a minimum of 10 days prior to requested cancellation date. I am responsible for paying for services in a timely matter and account balances left unpaid past 60 days will be referred to an outside collection agency for handling. I consent to start weekly pool service with Vistancia Pool Care.
